Capital's streets very lively due to the atmosphere of the circumcision season

Every year, as Madagascar enters the dry season, the streets of the capital take on a unique festive atmosphere. To an unfamiliar visitor, the lively processions might resemble weddings, village festivals, or religious celebrations. Groups of people move through the streets singing, dancing, beating drums, and playing lively brass music. Long stalks of sugar cane are carried proudly on shoulders, while relatives cheer as a young boy, often dressed in his finest clothes, rides high above the crowd on the shoulders of family members.

Although most Malagasy families today organize individual ceremonies rather than participating in collective rituals, the festive processions currently filling the streets of Antananarivo still evoke the same communal spirit. Songs echo through neighborhoods. Brass bands animate the streets. Traditional dances accompany smiling families. Children wave sugar cane in celebration. For several weeks each year, the capital itself becomes a living expression of Malagasy heritage.

These joyful scenes are the unmistakable signs of one of Madagascar's most enduring traditions: didimpoitra, the Malagasy circumcision ceremony. Known in different regions as hasoavan-jazalahy or famoràn-jaza, the ceremony remains one of the country's most important rites of passage, transforming ordinary city streets into colorful stages where culture, family, and identity come together.

A Tradition Shared

Few customs unite Madagascar's remarkably diverse ethnic communities as strongly as circumcision. Although each region celebrates the event differently — with varying names, rituals, music, and festivities — the underlying purpose remains the same: marking a boy's transition into a new stage of life.

For generations, circumcision was performed according to ancestral customs by the Rainjaza, a respected traditional circumciser entrusted with carrying out the ritual. The procedure itself formed only one part of a much larger ceremony involving blessings, family gatherings, and community participation.

Today, however, Malagasy families celebrate the occasion in many different ways. Some still organize lavish festivities lasting several days, inviting neighbors, relatives, and entire communities to participate. Others have adopted a much simpler approach, limiting the celebration to close family members while preserving its symbolic meaning.

On the outskirts of Antananarivo, one family recently chose a completely private celebration. "We invited nobody," explained the grandmother of a little boy who had just undergone the procedure. There was no large feast, no overnight celebration, and no elaborate ceremony.

Early in the morning, the child — less than two years old — was taken to a medical clinic. During the operation, three men gently held him still while a female physician performed the circumcision.

"The procedure lasted less than a minute," his grandmother recalled. "He barely noticed what happened." The child underwent what many Malagasy refer to as an "American circumcision," a modern surgical technique widely practiced in medical clinics throughout the country. The operation cost approximately 60,000 ariary, although fees vary depending on the healthcare facility and the surgical method used. Stories like this illustrate how Malagasy families increasingly combine cultural traditions with modern medical care.

Medical Benefits

While circumcision occupies a central place in Malagasy culture, it is also recognized internationally as a legitimate medical procedure under specific circumstances. Circumcision involves removing part or all of the foreskin covering the head of the penis. In many cases, it is performed for medical reasons rather than cultural ones.

Doctors commonly recommend circumcision to treat phimosis, a condition in which the foreskin cannot be retracted properly, causing pain, repeated infections, or difficulty urinating. The operation may also become necessary in cases of paraphimosis, a medical emergency where the foreskin becomes trapped behind the glans, restricting blood circulation. Repeated episodes of balanitis, posthitis, or balanoposthitis — inflammations affecting the foreskin or glans — can likewise justify surgical treatment.

Scientific research has also shown additional health benefits. Circumcision reduces the likelihood of urinary tract infections in young boys, although these infections are relatively uncommon. Several large international studies have further demonstrated a reduced risk of acquiring certain sexually transmitted infections, including HIV during heterosexual intercourse in high-prevalence settings, human papillomavirus (HPV), and, to a lesser extent, genital herpes.

Medical professionals emphasize, however, that these protective effects are only partial. Circumcision does not replace safe sexual practices or condom use, which remain the most effective means of preventing sexually transmitted infections. Early circumcision has also been associated with a lower risk of penile cancer — a rare disease — and may indirectly reduce HPV transmission to female partners.

For Malagasy families, circumcision represents one of life's defining milestones. It symbolizes courage, maturity, and the gradual integration of a young boy into the community of men.
